- Na hřbetě štítek s poškozeným rukopisným titulem rukou 17. století De 3 regibus (?) et B. Grego. Scala (?) claustralium, mladší rukou zde doplněno Vita Ven. Arnesti, arch. Pragensis. Na spodní části desek fragmenty dalších stržených štítků. Potah předního přídeští z větší části stržen, částečně dochován poškrábaný obsah rukopisu rukou 14.-15. století a starší klementinská signatura XIV.D.6. Podle B. Balbína byl rukopis uchováván v jezuitské koleji v Českém Krumlově.
